Tremor‑dominant presentations feature a prominent resting tremor and are often associated with slower progression and relatively preserved cognition. These patients frequently retain independence for longer and respond well to standard treatments.
The postural instability and gait difficulty (PIGD) subtype is characterised by balance problems, falls and freezing of gait. This pattern usually carries a higher risk of earlier disability and greater cognitive decline.
Akinetic‑rigid presentations show predominant bradykinesia and rigidity with minimal tremor; progression and cognitive risk tend to be intermediate between tremor‑dominant and PIGD forms. Mixed phenotypes are common and symptom profiles can evolve over time.
Young‑Onset Parkinson’s Disease
Young‑onset Parkinson’s disease, with symptom onset before age 50, accounts for a minority of cases. Genetic factors are more frequently implicated in this group. Although progression may be slower and levodopa responsiveness preserved, motor complications such as dyskinesias often appear earlier because of longer treatment exposure.
Psychosocial impacts — including effects on employment, finances and family planning — are particularly important in younger people and merit timely specialist support and practical advice.
